You may purchase up to 15 luncheon tickets on this form – for yourself and others. When prompted, provide the names of the attendees, if known. If not, use your name as a placeholder/ticketholder. For parties larger than 15, please have another person complete additional form(s).  There is a 15 ticket purchase limit per person. You will pick up your ticket at Roslyn Heights.

TERMS and CONDITIONS (required reading):

Mobility: Roslyn Heights was built in 1895 and is on the National Register of Historic Places. Please be aware that in order to gain access to the building, attendees must climb a set of stairs to enter the first floor. MSSDAR does not provide physical assistance into the home and is not equipped to assist with entry to the building, or assist with navigating throughout the three floors. An electric chair lift is available for attendees with mobility concerns to access the second level.
